What You Should Know Before Your Aviation Examination

MEDXPRESS APPLICATION

In order to process your certification request, please complete the FAA's MedXPress electronic application prior to your appointment, and bring a printed copy of the FAA Form 8500-8 generated from submission of the MedXPress application with you to your appointment.  Your application cannot be processed without the confirmation number stamped on the bottom of Form 8500-8.

mEDICAL RECORDS

To avoid delays in the issuance of your medical certificate please ensure to bring adequate documentation about your medical condition(s) with you to your appointment. For more information on what conditions warrant supporting documentation please reference the following resource link below.

FAA LETTERS

If you have received a Special Issuance Medical Certification Authorization and/or Statement of Demonstrated Ability (SODA) letter from the FAA for a disqualifying condition, please bring the letter(s) and required supporting documentation with you to your appointment.

EXAMINATION

During your appointment we will be conducting a urinalysis and eye exam.  As a result, please come prepared to provide us with a urine sample and bring with you any and all corrective lenses you wear.
