In order to process your certification request, please complete the FMCSA's , Medical Examination Report (MER) form MCSA-5875 and bring a printed copy of the form with you to your appointment.  Your application cannot be processed without the MER form.

To avoid delays in the issuance of your medical certificate please ensure to bring adequate documentation about your medical condition(s) from your treating physician with you to your appointment.  This includes  

If you possess a Special Performance Evaluation (SPE) Certification, vision, hearing, seizure, and/or Diabetes exemption, please ensure you bring the respective documentation with you to your appointment.  Failure to do so may result in a delay in the issuance of your medical certificate

During your appointment we will be conducting a urinalysis, eye exam, and hearing exam.  As a result, please come prepared to provide us with a urine sample, bring with you any and all corrective lenses you wear, and a copy of your Audiogram results if you wear hearing aids.
